JHS - Covid Vaccine Community

Thank you for your interest in receiving the COVID-19 vaccine at Jackson Health System. There is no charge for this vaccine at Jackson.

At this time, Jackson's vaccine supply is small and subject to change at any time. We may stop taking appointments at any time based upon our capacity. We appreciate your patience as we support the effort to vaccinate our community as quickly and safely as possible.

Currently, Jackson is exclusively providing the COVID-19 vaccine manufactured by Pfizer. You will be registered for a follow-up appointment to receive the second shot 21 days after your first. The vaccine is only fully effective after receiving both shots, and we require your commitment to receive both at Jackson. If we are providing a different approved COVID-19 vaccine on the day of your first dose appointment, you will receive additional information at that time.

Both doses of the vaccine must be administered by Jackson. We are unable to provide a second dose for patients who received the first dose from another provider.

When we schedule your appointment, it will be only for you. We are unable to vaccinate family members, others who live in your household, or other people during your appointment.

Jackson is vaccinating patients as young as 16 years old. Please answer all the following questions to find the right criteria for you.

Are you currently 18 or older?

 

Are you currently 50 or older and working as a law-enforcement officer, firefighter, or K-12 school employee?

 

Are you currently 18 or older, and has your doctor told you get vaccinated due to your high-risk condition?

 

Are you the parent or legal guardian of a 16- or 17-year old, and have you spoken to the patient's doctor about whether the COVID-19 vaccine is safe for them?

 

Will you or another parent or legal guardian of this patient be present at the appointment to provide consent?

 

Please type your name here to sign, acknowledge, and certify that your doctor has recommended COVID vaccination due to your high-risk condition. The name you enter must match your legal full name. No further medical documentation or doctor's note will be required at your appointment.

Please bring employee ID or other proof of workplace to your appointment.

Have you spoken to your doctor about whether the COVID-19 vaccine is safe for you?

 

Do you have a valid Florida driver's license or Florida identification card?

 

Do you live - either fulltime or parttime - in Florida?

 
Can you bring a photo ID with your date of birth and TWO of the following documents to your appointment, both matching the name on your photo ID:

  1. Valid student ID from a Florida college or university (no second document needed for college students – student ID and photo ID with date of birth are sufficient)
  2. Deed, mortgage, monthly mortgage statement, mortgage payment booklet, or residential rental or lease agreement
  3. A utility hookup or work order dated within 60 days
  4. A utility bill, no more than two months old
  5. Mail from a financial institution, including checking, savings, or investment account statements, not more than two months old
  6. Mail from a federal, state, county, or local government agency, not more than two months old

 

The COVID-19 vaccine has been found to be safe and effective for the vast majority of people. Because the vaccine is so new, however, we recommend discussing vaccination with your doctor and then coming back to this link.

At this time, you are not eligible for vaccination at Jackson. Please continue to monitor our website as criteria change.

Patients under 18 must have a parent or guardian present to provide consent.

The State of Florida is only permitting Jackson to vaccinate Florida residents at this time.

The State of Florida currently requires these documents to prove your residency. We cannot provide an appointment for you at this time.
