JHS - Covid Vaccine Community

Thank you for your interest in receiving the COVID-19 vaccine at Jackson Health System. There is no charge for this vaccine at Jackson.

At this time, Jackson's vaccine supply is small and subject to change at any time. We may stop taking appointments at any time based upon our capacity. We appreciate your patience as we support the effort to vaccinate our community as quickly and safely as possible.

Currently, Jackson is exclusively providing the COVID-19 vaccine manufactured by Pfizer. You will be registered for a follow-up appointment to receive the second shot 21 days after your first. The vaccine is only fully effective after receiving both shots, and we require your commitment to receive both at Jackson. If we are providing a different approved COVID-19 vaccine on the day of your first dose appointment, you will receive additional information at that time.

Both doses of the vaccine must be administered by Jackson. We are unable to provide a second dose for patients who received the first dose from another provider.

When we schedule your appointment, it will be only for you. We are unable to vaccinate family members, others who live in your household, or other people during your appointment.

Are you currently 65 or older?

 
Are you at least 50 years old employed with ID showing you work in one of the following professions?

  1. Sworn law-enforcement officer
  2. Firefighter
  3. School-site personnel in a K-12 school

 

Are you at least 50 years old and under a doctor's care for a high-risk condition?

 

You must bring a note signed by your doctor on the doctor's letterhead or prescription pad. This note must list the high-risk condition for which you are being treated AND must indicate that the doctor recommends you receive the COVID-19 vaccine. Will you bring this document to your appointment?

 
Are you at least 55 years old under a doctor's care for one of the following medical conditions?

  1. COPD
  2. End-stage renal failure
  3. Cardiomyopathy
  4. Congestive heart failure
  5. Coronary artery disease with bypass
  6. Sickle cell disease
  7. Down's Syndrome (guardian must be present and present proof of guardianship)
  8. Solid organ transplant recipient
  9. Morbid obesity, defined as having a body mass index above 40
  10. Lung cancer
  11. Breast cancer
  12. Leukemia
  13. Lymphoma

 

You must bring a doctor's note to your appointment. This note must be on your physician's letterhead or prescription pad and specify your name, the condition for which you are being treated AND must indicate that the doctor recommends you receive the COVID-19 vaccine. It also must be signed by the doctor. Will you bring this document to your appointment?

Do you live - either fulltime or parttime - in Florida?

 
Can you bring a photo ID with your date of birth and TWO of the following documents to your appointment, both matching the name on your photo ID:

  1. Deed, mortgage, monthly mortgage statement, mortgage payment booklet, or residential rental or lease agreement
  2. A utility hookup or work order dated within 60 days
  3. A utility bill, no more than two months old
  4. Mail from a financial institution, including checking, savings, or investment account statements, not more than two months old
  5. Mail from a federal, state, county, or local government agency, not more than two months old

 

Vaccination is currently only available for patients 65 and older. Updated information will be provided at JacksonHealth.org as Florida's vaccination program expands.

The COVID-19 vaccine has been found to be safe and effective for the vast majority of people. Because the vaccine is so new, however, we recommend discussing vaccination with your doctor and then coming back to this link.

Jackson is currently only providing vaccination for patients 65 and older, or those 55 and older with these conditions. Please check back in the future for updated criteria.

The State of Florida currently requires these documents to prove your residency. We cannot provide an appointment for you at this time.
